For information on admissions, enrollment, and necessary forms, please review below:

Admissions Criteria
  • Registered active Catholic families with other children presently enrolled in our school;

  • Registered active Catholic families without other children presently enrolled in our school;

  • Active Catholic families not living in Holy Angels parish whose home parish has no Catholic school (with permission of pastor);

  • Non-practicing Catholic families living within the boundaries of Holy Angels Parish School.

Enrollment Forms

If your child will be new to Holy Angels, please Click Here. Upon completion please bring the following information to the office:

  • Confirmation page

  • $150 registration fee

  • Birth Certificates

  • Sacramental Certificates - Baptism, First Communion, Confirmation

  • Vaccination record

  • Physical  if entering Kindergarten, 2nd, or 6th grades

  • Dental exam if entering Kindergarten, 2nd, or 6th grades

  • Eye Exam if entering Kindergarten

  • Records Request Form

Admissions Requirements
  • Children entering kindergarten must be five years of age on or before September 1.

  • Baptismal and birth certificates must be submitted for children entering school for the first time. Originals will be returned - copies will be kept on file.

  • Illinois State Law requires that all students be in compliance with health guidelines including physical exams, immunizations, dental exams, and vision exams. Applicable grades are Preschool (physical), Kindergarten (physical-preschool exam does not meet requirements, dental, vision), 2nd grade (dental), 6th grade (physical, dental) and all new students to Holy Angels (see New Student's Important Health Requirements). Preferably forms will be turned in at the time of registration or if not, they must be on file by August 1. No one may begin school who is not in full compliance.

Registration Procedure

Pre-registration is held in late winter to determine the number of students who will be returning the following year. Registration for new families takes place shortly afterwards, usually in March. However, it is never too late! Contact Holy Angels now to see if there is space available.

 

A non-refundable $150.00 fee (Discounts available for early registration) is required per family when enrolling. On Fee Day, parents inform new teachers of health problems, etc. Arrangements are made for payment of assessment fees, and ordering of milk.

Transferring Students

Upon admission or transfer, the child must present a report card and a transfer slip from the school previously attended. We will request medical records from previous school and will notify you if records are missing.
